Guideline facts

Monthly share
Age-, membership-type (Direct or Essential), and IUA-banded household shares. Confirm the current Zion schedule. We do not publish a frozen floor.
Member responsibility
Selectable IUA of $1,250, $2,500, or $5,000 per eligible sharing request. Households pay no more than three IUAs in a rolling 12-month period. Maternity uses a separate IUA in the maternity section.
Pre-existing
24-month lookback. One-year wait, then phase-in to $125,000 per request by year four and $125,000 per 12 months thereafter. Controlled hypertension, cholesterol, and type 1/2 diabetes can be excepted if not hospitalized in the prior 12 months.
Waiting periods
Pre-membership clock is one year. Preventive sharing has a six-month wait. Lowering an IUA starts a 60-day wait.
Maternity
Shareable under the maternity section with a structured maternity IUA. Newborns from an eligible maternity request follow a separate add-on rule.
Limits
Pre-membership phase-in caps apply. After year four, pre-membership sharing resets at $125,000 per membership year.
Cancellation
Membership timing is not federal Open Enrollment.
