Program facts
- Monthly share
- Age-, membership-type (Direct or Essential), and IUA-banded household shares. Confirm the current Zion schedule. We do not publish a frozen floor.
- Member responsibility (IUA)
- Selectable IUA of $1,250, $2,500, or $5,000 per eligible sharing request. Households pay no more than three IUAs in a rolling 12-month period. Maternity uses a separate IUA in the maternity section.
- Household pricing
- Individual, couple, and family-style tiers. Direct vs Essential changes preventive sharing.
- State availability
- Published exclusion: Washington. See Zion’s Washington Statement in the January 2026 guidelines.
- Pre-existing conditions
- 24-month lookback. One-year wait, then phase-in to $125,000 per request by year four and $125,000 per 12 months thereafter. Controlled hypertension, cholesterol, and type 1/2 diabetes can be excepted if not hospitalized in the prior 12 months.
- Waiting / phase-in
- Pre-membership clock is one year. Preventive sharing has a six-month wait. Lowering an IUA starts a 60-day wait.
- Maternity
- Shareable under the maternity section with a structured maternity IUA. Newborns from an eligible maternity request follow a separate add-on rule.
- Preventive
- Direct includes specified preventive sharing after a wait, not subject to the IUA. Essential needs the Preventive Sharing add-on. Not an ACA mandate unless bundled with a MEC (HSA Secure).
- Prescriptions
- Program-specific. Confirm the current unshareable list in the January 2026 book.
- Virtual care
- Advertised on the public site. Confirm current partners.
- Provider access
- Typically any licensed U.S. provider. Sharing is not a network contract. Bills must be submitted within six months.
