Program facts

Monthly share
Household size, oldest member’s age, and chosen annual PRA. Confirm the current Impact calculator — we do not freeze a monthly number.
Member responsibility (IUA)
Annual household Primary Responsibility Amount, then 10% co-share until $5,000 of co-share is paid. PRA options include $2,500 / $5,000 / $7,500 / $10,000 (and $1,000 for members 65+).
Household pricing
One PRA for the household for the membership year. Not per person.
State availability
Confirm current geographic rules.
Pre-existing conditions
36-month lookback. A condition is shareable only after 36 consecutive months without signs, symptoms, testing, diagnosis, treatment, or medication. Controlled hypertension/cholesterol are excepted for later vascular/cardiac events.
Waiting / phase-in
First 60 days capped at $50,000 (excluding pre-existing). Maternity needs a 10-month estimated due date.
Maternity
EDD at least 10 months after the mother’s start date. $150,000 cap per pregnancy event including complications to the mother.
Preventive
Annual well visit and a $150 lab allowance share at 100% without PRA. Other preventive screening is subject to PRA and co-share. Not an ACA mandate.
Prescriptions
May credit toward PRA if not treatment for a pre-existing condition. After PRA, sharing is limited ($1,200 per member per year in the June 2026 book).
Virtual care
Urgent-care telemedicine with a $0 provider fee is described as starting the first month.
Provider access
No network. Impact is always secondary to other coverage, including auto and workers’ comp.
Limits
$500,000 per member per membership year. Motorcycle incidents $100,000.
Membership requirements
Statement of Shared Beliefs & Ethics. Illegal-drug abstinence. EFT required. Age 65+ must have Medicare A & B (Part D for Rx).
Cancellation
Guidelines in force on the date of service govern, not the book in force when the member joined.
